How to Organize Your Family Medical History in Wingspan Health
What is Family Medical History?
Your family medical history is a record of health conditions in your family from three generations including you, your parents, and your grandparents. Typically, you would fill this out at your first visit to your doctor in a form.
Why do you need to know your family medical history?
To see if you have a higher-than-usual chance of having certain health conditions.
To help your doctor decide on early screening for conditions you may be at an increased risk.
To learn more about your family!
What is the Family History feature in Wingspan Health?
The Family History feature gives you an online medical history of your family's health conditions in your Wingspan Health account.
Features:
Family History Questionnaire: make note of the conditions your family has through a comprehensive questionnaire (currently your parents and grandparents on both sides).
Ask a Family Member: send a custom link to a family member to complete the family history questionnaire for any particular family member.
Family History Tree: see all of the recorded conditions for each family member in an interactive diagram.
Family History Pivot Table: get a categorized summary of all the health conditions your family has where you can see who has what conditions and the whether those conditions may increase your risk.
How does the Family Medical History feature in Wingspan work?
Make note of your family's health conditions with the Family History Questionnaire.
The Family History Questionnaire is a comprehensive form in Wingspan Health's Family History feature that asks if a particular family member has a condition(s) among a list for 17 health categories:
How to take the Family History Questionnaire in Wingspan Health:
Sign up for a Wingspan Health account.
Log into your Wingspan Health account.
Click the Family History block on the Dashboard OR click on Family History in the menu on the left.
Click on a block in the Family History Tree to fill out the questionnaire for a family member OR click the Ask a Family Member button to send the questionnaire for a family member to fill out.
You will be shown a list of conditions for a particular health category and will have to check off the ones that your particular family member has. If you do not see their condition listed, you can always add it in under Other by specifying.
When you reached the end of the questionnaire, hit the Submit button!
Ask a family member to fill out the Family History Questionnaire.
The Ask a Family Member feature allows another family member to fill out the Family History Questionnaire for your Family History in Wingspan Health through a custom link.
How your family member can fill out your Family History Questionnaire:
Click the Ask a Family Member button on the Family History page.
Select a family member in a dropdown menu to generate a custom link for that family member.
Copy a link for a particular family member in the box below the Family History Tree to send to a family member to fill out.
Send the link to a family member to fill out the Family History Questionnaire for the selected family member.
See all recorded health condition for each family member in a Family History Tree.
The Family History Tree is an interactive diagram that shows all of the recorded conditions from the Family History Questionnaire for you and all of your family members (mother, father, maternal grandmother, maternal grandfather, paternal grandmother, and paternal grandfather).
How to view each family members’ recorded conditions:
Go to the Family History page in your Wingspan Health account.
Hover over the family member who you'd like to see recorded conditions.
Get a categorized summary of all the health conditions your family has in a Family History Table.
The Family History Pivot Table is a summary of all of the recorded conditions from the questionnaire organized by 8 health categories:
Digestive
Allergies
Cancer
Neurological
Hearing
Vision
Respiratory
Diabetes
How to see the health categories and the recorded conditions where you might be at risk:
Go to the Family History page in your Wingspan Health account.
Look at the last column for Risk Relationship and see if a certain health condition may increase your risk.
